All added. In the case of a player not appearing before the first goal is scored, the bet is settled as a straight bet on the score at the appropriate price for that bet. This would have to be done manually and pay out may be delayed.
vBookie rules state that if you place a bet on a scorer who doesn't play, it's a losing bet. The same thing would apply in this case. So you should wait until the teams are announced before placing a bet if you're not sure if the player will start.
