It can't be cached on my computer because the Temporary Internet Files folder is emptied every time I close the browser. I have also Ctrl+F5'ed so I should now have a new copy, but the smilie is still there (Row3, column3)

Depends on how many fingers he's got. How can a smiley alone offend anyway? If it's accompanied by text which is offensive or abusive then action can be taken over that if a moderater thinks it deserves it.
